The proposed project develops and evaluates programmes that implement patient involvement instruments to improve general practice care for older people in Europe, resulting in an attractive toolkit of materials for general practitioners and patients in 11 European countries. Many instruments to measure patients' views and educate care providers have been developed but not implemented in clinical practice.
This project builds a toolkit in three steps:
1. An interview study to document and compare internationally the barriers for patient involvement in general practice care for older people.
2. Programme development and case studies to develop and select programmes that can help to implement patient involvement instruments in this area.
3 An international program evaluation to assess and compare internationally the most promising programmes.
