Due to rising costs in public health system, general practitioners (GPs) are faced with new demands: in Managed Care they should function as gatekeepers. They hold a key position relating to the referral of patients to specialists. In order to fulfil these important functions it is necessary for GPs to have at their disposal communication skills with enable them properly to identify the concerns of their patients. During recent years these communication skills have become increasingly an issue in the discussion about the quality of medical care and have also been better remunerated. The question now is how these communication skills can be taught to the GPs of future, or can be trained and tested by the GPs already in practice.
Method: The present research project has the objective to throw light upon doctor/patient communication within a comparative study in different European countries by the means of different instruments and ratings procedure already frequently employed in the Netherlands.
Data: In our project 20 regular consultations will each be video-recorded in the consulting room of 20 GPs in the German and 10 GPs in the French part of Switzerland. These video tapes will then be rated by a content analysis, by means of the 'Roter Interaction Analysis System' (RIAS) and a 'Patient-Centredness Instrument'. In addition there will be a structured questioning of patients and the doctors before and after the consultation.
Questions: Our empirical work will inform us about the following questions:
1. Which determinants influence doctor/patient communication and in which way: social stratification, gender, age, nationality and reason of visit of the patient; gender of doctor and length of treatment?
2. Which communication models correspond to the communicative behaviour of the GPs involved: patient-centred vs. doctor-centred of affective vs. instrumental?
Results: If, as to be expected from the literature, the anticipated results
1. communication obstacles are above all due to social stratification or gender and
2. doctor's communication style shape content, outcome and patient satisfaction
are found, it would be one of the goals of this project to show these findings to the interested physicians and to contribute to an improvement of the education and practice of counselling/communications skills of today's and future GPs. At the same time, the comparison of Swiss results of the study with the results from the other countries should throw light upon the question of whether different public health systems have an impact on doctor/patient communication.
