Die medizinische Bildgebung ist ein wesentlicher Bestandteil der modernen Medizin. Mit 2,1 Milliarden Franken entfiel 2023 ein Sechstel des ambulanten Leistungsvolumens in der obligatorischen Krankenversicherung (KVG) auf die Bildgebung. Dabei sind die Kosten in den letzten Jahren durchschnittlich um mehr als 5 % gestiegen. Der hohe Verbrauch steht im Zusammenhang mit der Pro-Kopf-Dichte von Computertomografen (CT) und Magnetresonanztomografen (MRT) in der Schweiz, welche die höchste in Europa ist.

Der Einsatz von medizinischen Bildgebungsverfahren ist im KVG praktisch nicht geregelt. Die Ärztinnen und Ärzte entscheiden selbst, ob sie solche Verfahren einsetzen wollen, und richten sich dabei nach den von internationalen Ärzteorganisationen herausgegebenen Empfehlungen. Wie jede Leistung der obligatorischen Krankenversicherung muss jedoch auch der Einsatz von medizinischer Bildgebung den Kriterien der Wirksamkeit, Zweckmässigkeit und Wirtschaftlichkeit (WZW) entsprechen. Die Einhaltung dieser Kriterien muss von den Krankenversicherern anhand der Rechnungsdaten kontrolliert werden.

Die Eidgenössische Finanzkontrolle (EFK) hat den Einsatz der wichtigsten Bildgebungsverfahren in einzelnen Fällen analysiert und zieht eine durchzogene Bilanz betreffend den derzeitigen Rahmen für die Bildgebung. So führt das liberale Umfeld zum Beispiel zu grossen Unterschieden, sei es innerhalb der Ärzteschaft bei Schwangerschaftsultraschallen oder zwischen den Kantonen bei CT- und MRT-Untersuchungen.

Die Versicherer kontrollieren den Einsatz medizinischer Bildgebung nur unzureichend

Die Kontrolle der Angemessenheit der gemäss KVG vergüteten Leistungen liegt vollständig in der Verantwortung der Krankenversicherungen. Die im ambulanten Bereich eingesetzten Kontrollinstrumente sind insbesondere darauf ausgerichtet, mit statistischen Analysen Leistungserbringerinnen und Leistungserbringer zu ermitteln, deren Preispraktiken und durchschnittliche Einnahmen pro versicherte Person von der Norm abweichen. Trotz ihres erheblichen Kostenanteils wird die medizinische Bildgebung insgesamt nur wenig in diese Kontrollen einbezogen: Der vorschriftsmässige Einsatz von Grossgeräten für die medizinische Bildgebung in den Radiologieinstituten wird nur vereinzelt überprüft, während die Verwendung von Ultraschall gar nicht überprüft wird.

Für CT- und MRT-Untersuchungen sollten die Verschreibungen der behandelnden Ärztinnen und Ärzte eine wichtige Kontrollkomponente bilden. Diese werden aber nicht geprüft, unter anderem aufgrund der mangelhaften Angaben zu den Verschreiberinnen und Verschreibern auf den Rechnungen. Die EFK empfiehlt dem Bundesamt für Gesundheit (BAG), bei den Tarifpartnern darauf hinzuwirken, dass auf den Rechnungen immer rückverfolgbare Angaben zu den Verschreiberinnen und Verschreibern sowie den Leistungserbringerinnen und Leistungserbringern enthalten sind, welche die Versicherer für ihre Kontrollen verwenden können. Sie empfiehlt dem BAG zudem, sich bei der medizinischen Bildgebung für eine Verbesserung der Kontrollen der Versicherer einzusetzen.

Die Auswirkungen der angekündigten Tarifrevision auf die Bildgebung müssen im Auge behalten werden

Zurzeit ermöglicht die gemäss Tarmed garantierte Vergütung den Betrieb von Grossgeräten für die medizinische Bildgebung zu günstigen Bedingungen. Die Gewinnschwelle kann auch dann erreicht werden, wenn die bei der Einführung des Tarifs vor knapp 25 Jahren übernommenen Effizienzstandards deutlich unterschritten werden.

Dieses günstige Tarifklima hat zur Entwicklung eines umfangreichen und komfortablen Angebots geführt, das es den Patientinnen und Patienten ermöglicht, ohne Wartezeiten und nahe am Wohnort behandelt zu werden.

Mit dem für 2026 vorgesehenen Inkrafttreten des neuen ambulanten Tarifs werden sich die Vergütungsbeträge für Leistungserbringerinnen und Leistungserbringer in der medizinischen Bildgebung ändern. Die geprüften Elemente lassen eine deutliche Reduktion des Tarifanteils vermuten, der für die Infrastrukturfinanzierung der Radiologieinstitute bestimmt ist. Die EFK empfiehlt deshalb dem BAG, die Auswirkungen der Einführung des neuen Tarifs genau zu verfolgen. Damit soll insbesondere sichergestellt werden, dass die neue Tarifierung nicht zu einem unerwünschten Anstieg der Anzahl Untersuchungen führt.

Kantonale Rahmenbedingungen für das Bildgebungsangebot haben wenig Auswirkungen

Die Kantone sind angesichts der zunehmenden Bedeutung der medizinischen Bildgebung nicht untätig geblieben. Die lateinischen Kantone haben aus eigener Initiative eine Bedürfnisklausel für medizintechnische Grossgeräte, darunter CT- und MRT-Geräte, eingeführt. Diese Klauseln haben aber nicht wie erwartet zu einer moderateren Verwendung von Bildgebungsverfahren geführt. Tatsächlich ist in den Kantonen, die solche Klauseln eingeführt haben, die Häufigkeit von CT- und MRT-Untersuchungen nach wie vor am grössten und nimmt auch am schnellsten zu.

Neue Bestimmungen im KVG verlangen, dass die Kantone ab Mitte 2025 eine Obergrenze für die Anzahl der Ärztinnen und Ärzte bestimmter Fachgebiete festlegen, wenn eine Überversorgung herrscht. Die Radiologie ist eines der medizinischen Fachgebiete, auf das die von mehreren Kantonen ausgearbeiteten Projekte bezüglich Obergrenzen abzielen. Da eine Besitzstandswahrung vorgesehen ist, dürfte die Wirkung dieser Obergrenze zunächst bescheiden bleiben und erst in einigen Jahren beurteilt werden können.

Audits im Strahlenschutz können den Rahmen für die Bildgebungspraktiken verbessern

Im Rahmen seiner Zuständigkeit für den Strahlenschutz steuert das BAG seit 2018 ein Projekt für klinische Audits für Betriebsstandorte von medizinischen Geräten mit intensiver Strahlung, darunter CT-Geräte. Die von Fachkolleginnen und -kollegen durchgeführten Audits sind das einzige Instrument, um anhand konkreter Fälle die Qualität der Verschreibungen und der Untersuchungen zu überprüfen. Auch wenn sich die Audits nur auf CT-Geräte beschränken, stellen sie trotzdem eine zusätzliche Massnahme für einen besseren Rahmen für die medizinische Bildgebung in der Schweiz dar. Derzeit wird die Umsetzung der Auditempfehlungen nicht systematisch überprüft und ihre Auswirkungen sind kaum messbar.

2025 wird der Bundesrat über den Stand der Fortschritte dieses Projekts informiert werden. Die EFK empfiehlt, ein wirksames Instrumentarium vorzusehen, um die Umsetzung der Empfehlungen durch die Gerätebetreibenden zu überprüfen.

Das elektronische Patientendossier als Instrument zur Effizienzsteigerung

Um Doppeluntersuchungen und gesundheitsschädliche Bestrahlungen zu vermeiden, sollten die Ärztinnen und Ärzten bei der Behandlung überprüfen, ob bei der zu untersuchenden Person bereits Bildmaterial vorhanden ist. Diese Überprüfung ist aufgrund des eingeschränkten Informationszugangs und der begrenzten Zeit, die dem medizinischen Personal zur Verfügung steht, nicht immer gänzlich möglich. Das elektronische Patientendossier (EPD), in dem die wichtigsten medizinischen Informationen enthalten sind, sollte dieses Problem lösen. Zurzeit ist das EPD aber noch nicht wirkungsvoll, da zwar die technischen Standards für die medizinische Bildgebung im EPD festgelegt wurden, aber weder radiologische Berichte noch Links zu Bildern im Dossier zu finden sind. Der Grund hierfür ist, dass nur wenige Versicherte und Leistungserbringerinnen und Leistungserbringer das EPD nutzen, solange dieses nicht obligatorisch ist.

Angesichts der offensichtlichen Effizienz- und Sicherheitsvorteile empfiehlt die EFK dem BAG, mit der angekündigten Totalrevision des EPD-Gesetzes (EPDG) einen vollständigen Zugang zu den Radiologieberichten und medizinischen Bildern über das Patientendossier zu gewährleisten.

Medical imaging is a key tool of modern medicine. At CHF 2.1 billion in 2023, imaging accounted for one sixth of the volume of outpatient benefits covered by compulsory health insurance (under the Health Insurance Act, or HIA), with costs rising by an average of more than 5% in recent years. This high level of consumption is related to Switzerland's per-capita density of computed tomography (CT) and magnetic resonance imaging (MRI) devices, which is the highest in Europe.

The use of medical imaging is hardly regulated by the HIA. The choice whether or not to use medical imaging is left to the discretion of doctors, who are guided by recommendations issued by international practitioners' associations. However, like any other compulsory health insurance benefit, the use of medical imaging must comply with the principles of effectiveness, appropriateness and economic efficiency (EAE). Compliance with these principles must be monitored by health insurers using billing data.

The Swiss Federal Audit Office (SFAO) analysed the use of the main imaging techniques for specific cases, resulting in a mixed assessment of the current supervision system. This liberal environment leads, for example, to significant differences, whether between doctors for pregnancy ultrasounds or between cantons for CT and MRI scans.

Insurers do not audit the use of medical imaging sufficiently

Auditing the compliance of benefits reimbursed under the HIA is the sole responsibility of health insurance companies. The control instruments implemented in the outpatient sector aim in particular to identify, through statistical analyses, suppliers with tariff practices and average revenues per insured person that deviate from the norm. Despite its financial significance, controls of medical imaging are few and far between: the proper use of 'heavy' medical imaging devices is checked occasionally with radiology institutes, while ultrasound imaging devices are never checked.

For the use of CT and MRI, the prescriptions of treating physicians should be a vital dimension of control. However, they are not analysed, in particular because of the poor quality of data relating to prescribers on invoices. The SFAO recommends that the Federal Office of Public Health (FOPH) call on tariff partners to ensure that traceable data concerning the prescriber and the supplier is always available on invoices and usable for insurers' checks. The SFAO also recommends that the FOPH take action to improve insurers' checks on the use of medical imaging.

The effects of the announced tariff revision on imaging must be monitored

The remuneration provided by the TARMED tariff structure now means that heavy imaging devices can be operated on favourable terms. Profitability thresholds can be achieved while remaining well below the efficiency standards adopted when the tariff was created nearly 25 years ago. This favourable tariff situation has led to the development of an abundant and convenient range of solutions, which allows patients to be examined without delay and close to their home.

The entry into force of the new outpatient tariff planned for 2026 will mean changes to the amounts reimbursed to medical imaging providers. Based on the elements examined, it should lead to a significant reduction in the tariff share reserved for financing the infrastructure of radiological institutes.

In this light, the SFAO recommends that the FOPH monitor the impact of the introduction of the new tariff, in particular to ensure that it does not lead to an unwanted increase in the number of examinations.

Cantonal supervision of imaging services has little effect

The cantons have responded to the growing importance of medical imaging. The cantons in Romance language-speaking Switzerland have introduced, on their own initiative, a need clause for heavy medical technical equipment, including CT and MRI scanners. However, these clauses have not had the anticipated effect of reducing the use of medical imaging, as in the cantons which have introduced such clauses the frequency of CT and MRI scans remains the highest and is increasing the fastest.

New HIA provisions require that cantons apply, from mid-2025, a cap on the number of doctors practising in specialisms with an overabundant supply. Radiology is one of the medical disciplines targeted by the plans for caps developed by a number of cantons. As existing entitlements are expected to be retained, the effect of this cap is likely to remain modest at first, and will only be assessable in some years' time.

Radiation protection audits are likely to improve the supervision of imaging practices

As part of its expertise in radiation protection, the FOPH has been piloting a clinical audit project since 2018 covering sites operating intense-radiation medical devices, including CT scanners. These peer-conducted audits are the only instrument for checking the quality of prescriptions and the roll-out of examinations based on specific cases. Although they only relate to CT scans, these audits constitute an additional measure which could lead to better supervision of medical imaging in Switzerland. Currently, the implementation of audit recommendations is not systematically monitored and their effect is difficult to measure.

The Federal Council will be informed in 2025 of the progress in implementing this project. The SFAO recommends providing an effective system for verifying the implementation of recommendations by device operators.

The electronic patient record as a tool for efficiency gains

A treating physician is expected to check whether the patient has already had imaging in order to avoid duplication and radiation that is harmful to their health. Such checks are not always successful owing to difficulties in accessing information and the limited time available to doctors. The system of electronic patient records (EPRs) aimed at centralising key medical documents should solve this problem, but it is currently not being used to full effect. The technical standards for medical imaging in the EPR have been defined, but neither radiology reports nor links to images are contained in the EPR, as insured persons and suppliers are not required to participate.

Given the obvious efficiency and security gains, the SFAO recommends that, as part of the announced total revision of the Electronic Patient Record Act (EPRA), the FOPH ensure full access to radiology reports and medical images through patient records.

L’imagerie médicale constitue un instrument essentiel de la médecine moderne. Avec 2,1 milliards de francs en 2023, l’imagerie représentait un sixième du volume des prestations ambulatoires relevant de l’assurance-maladie obligatoire (LAMal), avec une hausse des coûts de plus de 5 % en moyenne ces dernières années. Cette consommation élevée est à mettre en relation avec la densité par habitant en Suisse des appareils de tomodensitométrie (CT) et des appareils à résonnance magnétique (IRM) qui est la plus élevée d’Europe.

Le recours à l’imagerie médicale n’est pratiquement pas régulé par la LAMal. Le choix du recours à l’imagerie médicale est laissé à la libre appréciation des médecins, lesquels s’orientent grâce à des recommandations émises par des associations internationales de praticiens. Comme toute prestation de l’assurance obligatoire des soins, l’usage de l’imagerie médicale doit toutefois respecter les principes d’efficacité, d’adéquation et d’économicité (EAE). Le respect de ces principes doit être contrôlé par les assureurs-maladie à partir des données de facturation.

Le Contrôle fédéral des finances (CDF) a analysé l’emploi des principales techniques d’imagerie pour des cas particuliers et tire un bilan mitigé du système d’encadrement actuel. Cet environnement libéral débouche par exemple sur des différences importantes, que ce soit entre médecins pour les ultrasons de grossesse ou entre cantons pour les CT et les IRM.

Les assureurs ne contrôlent pas suffisamment l’usage de l’imagerie médicale

Le contrôle de la conformité des prestations remboursées par la LAMal incombe entièrement aux sociétés d’assurance-maladie. Les instruments de contrôle mis en place sur le secteur ambulatoire visent en particulier à identifier, par des analyses statistiques, les fournisseurs avec des pratiques tarifaires et des revenus moyens par assurés s’écartant de la norme. Malgré son poids financier, le domaine de l’imagerie médicale est globalement peu couvert par ces contrôles : l’usage conforme de l’imagerie médicale lourde est vérifié ponctuellement auprès des instituts de radiologie alors que celui des ultrasons ne l’est jamais.

Pour l’emploi des CT et les IRM, les prescriptions des médecins traitants devraient constituer une dimension essentielle du contrôle. Or, elles ne sont pas analysées, notamment en raison de la mauvaise qualité des données relatives aux prescripteurs sur les factures. Le CDF recommande à l’Office fédéral de la santé publique (OFSP) d’intervenir auprès des partenaires tarifaires afin que la traçabilité des données concernant le prescripteur et le fournisseur soient toujours disponibles sur les factures et exploitables pour les contrôles des assureurs. Il lui recommande également d’intervenir afin que les contrôles des assureurs portant sur l’usage de l’imagerie médicale soient améliorés.

Les effets de la révision tarifaire annoncée sur l’imagerie doivent être suivis

La rémunération garantie par Tarmed permet aujourd’hui l’exploitation d’appareils d’imagerie lourde à des conditions favorables. Les seuils de rentabilité peuvent être atteints en restant bien en-dessous des normes d’efficience retenues lors de la création du tarif, il y a près de 25 ans. Ce contexte tarifaire favorable a conduit au développement d’une offre abondante et confortable, ce qui permet aux patients d’être examinés sans délai et à proximité de leur domicile.

L’entrée en vigueur du nouveau tarif ambulatoire prévu pour 2026 modifiera les montants remboursés aux prestataires d’imagerie médicale. Selon les éléments examinés, il devrait conduire à une diminution sensible de la part tarifaire réservée au financement de l’infrastructure des instituts de radiologie.

Le CDF recommande dès lors à l’OFSP de suivre l’impact de l’introduction du nouveau tarif, notamment afin de s’assurer qu’il ne conduise pas à une augmentation non souhaitée du nombre d’examens.

L’encadrement cantonal de l’offre d’imagerie a peu d’effets

Les cantons ne sont pas restés inactifs face à l’importance croissante de l’imagerie médicale. Les cantons latins ont introduit de leur propre initiative une clause du besoin sur les appareils médicotechniques lourds, dont les CT et les IRM. Ces clauses n’ont pas eu l’effet modérateur attendu sur la consommation d’imagerie médicale. C’est en effet dans les cantons ayant introduit de telles clauses que la fréquence des CT et des IRM reste la plus élevée et c’est également là qu’elle augmente le plus rapidement.

De nouvelles dispositions de la LAMal exigent que les cantons appliquent dès mi-2025 un plafonnement du nombre des médecins exerçant dans des spécialités connaissant une offre surabondante. La radiologie est l’une des disciplines médicales ciblées par les projets de plafonnement élaborés par plusieurs cantons. Comme il est prévu que les droits acquis soient garantis, l’effet de ce plafonnement devrait rester initialement modeste et ne pourra être évalué que dans plusieurs années.

Les audits de radioprotection sont susceptibles d’améliorer l’encadrement des pratiques d’imagerie

Dans le cadre de ses compétences en matière de radioprotection, l’OFSP pilote depuis 2018 un projet d’audits cliniques portant sur les sites exploitant des appareils médicaux à rayonnement intense, dont les CT. Ces audits, conduits par des pairs, constituent l’unique instrument de contrôle de la qualité des prescriptions et du déroulement des examens sur la base de dossiers concrets. Même s’ils ne portent que sur les CT, ces audits constituent une mesure supplémentaire pouvant conduire à un meilleur encadrement de l’imagerie médicale en Suisse. Actuellement, la mise en œuvre des recommandations d’audit ne fait l’objet d’aucun suivi systématique et leur effet est difficilement mesurable.

Le Conseil fédéral sera informé en 2025 de l’état d’avancement de la mise en œuvre de ce projet. Le CDF recommande d’y prévoir un dispositif efficace de vérification de l’implémentation des recommandations par les exploitants d’appareils.

Le dossier électronique du patient comme instrument de gain d’efficience

Il est attendu d’un médecin traitant qu’il vérifie si le patient dispose déjà d’imagerie afin d’éviter des doublons et des irradiations nocives à sa santé. Cette recherche n’aboutit pas toujours en raison des difficultés d’accès et du temps réduit dont disposent les médecins. Le dossier électronique du patient (DEP) visant à centraliser les documents médicaux les plus importants devrait résoudre ce problème mais il est inopérant aujourd’hui. Les standards techniques relatifs à l’imagerie médicale dans le DEP ont été définis, mais ni les rapports de radiologie, ni les liens aux images ne s’y trouvent faute de participation des assurés et des fournisseurs tant qu’il n’est pas obligatoire.

Vu les gains d’efficience et de sécurité évidents, le CDF recommande à l’OFSP de garantir une accessibilité complète aux rapports de radiologie et aux images médicales à travers le dossier du patient à l’occasion de la révision totale annoncée de la loi sur le DEP (LDEP).

La diagnostica per immagini (imaging) costituisce uno strumento fondamentale della medicina moderna. Con 2,1 miliardi di franchi nel 2023, l’imaging rappresenta un sesto del volume delle prestazioni nel settore ambulatoriale coperte dall’assicurazione malattia obbligatoria secondo la legge federale del 18 marzo 1994 sull’assicurazione malattie (LAMal; RS 832.10) e ha registrato un aumento medio dei costi superiore al cinque per cento rispetto agli anni precedenti. Il vasto impiego è da ricondurre alla densità delle apparecchiature per tomografia computerizzata e risonanza magnetica per abitante in Svizzera, che è la più elevata in Europa.

La LAMal non prevede praticamente alcuna disposizione del ricorso all’imaging. I medici possono decidere liberamente, sulla base delle raccomandazioni emanate dalle associazioni internazionali di categoria. Tuttavia, come per tutte le prestazioni previste dall’assicurazione obbligatoria delle cure medico-sanitarie, anche il ricorso all’imaging deve rispettare i criteri di efficacia, appropriatezza e economicità (noti come «criteri EAE»). Il rispetto di tali criteri deve essere controllato dagli assicuratori malattie sulla base dei dati di fatturazione.

Il Controllo federale delle finanze (CDF) ha esaminato l’impiego delle principali tecniche di imaging in alcuni casi particolari e ha stilato un bilancio piuttosto sfavorevole sull’attuale sistema di assistenza. L’indulgente approccio descritto in precedenza porta, ad esempio, ad alcune importanti differenze, soprattutto tra i medici che eseguono ultrasonografie durante la gravidanza o tra i Cantoni per quanto concerne il ricorso a tomografie computerizzate e risonanze magnetiche.

Gli assicuratori non controllano a sufficienza il ricorso all’imaging

La responsabilità dei controlli delle prestazioni coperte dalla LAMal compete esclusivamente alle società di assicurazione malattia. Gli strumenti di controllo impiegati nel settore ambulatoriale mirano, in particolare, a identificare, mediante analisi statistiche, i fornitori che applicano tariffe e percepiscono redditi medi per assicurato che si discostano dallo standard. Nonostante la sua importanza a livello finanziario, il settore dell’imaging non è generalmente sottoposto a questi controlli: il corretto impiego di tale diagnostica medica pesante viene puntualmente verificato presso gli istituti di radiologia, mentre quello dell’ultrasonografia non lo è mai.

Le prescrizioni dei medici curanti dovrebbero costituire un parametro di controllo fondamentale dell’utilizzo di apparecchi per tomografie computerizzate e risonanze magnetiche. Al momento, però, tali prescrizioni non vengono esaminate, soprattutto per via della scarsa qualità dei dati sulle fatture relativi a chi prescrive le prestazioni. Il CDF raccomanda all’Ufficio federale della sanità pubblica (UFSP) di intervenire presso i partner tariffali affinché sulle fatture sia possibile tracciare i dati di chi prescrive e di chi fornisce le prestazioni e, pertanto, effettuare i controlli sugli assicurati. Il CDF raccomanda anche di intervenire perché vengano migliorati i controlli degli assicurati sul ricorso all’imaging.

Necessità di monitorare gli effetti della revisione tariffaria prevista per l’imaging

Ad oggi, la retribuzione garantita da Tarmed consente di usufruire di apparecchiature per l’imaging pesante a condizioni favorevoli. Le soglie di redditività possono essere raggiunte restando ben al di sotto degli standard di efficienza stabiliti al momento della creazione della tariffa, che risale a circa 25 anni fa. L’effetto di questo contesto tariffario positivo è lo sviluppo di un’offerta abbondante e «comoda» che consente ai pazienti di essere visitati in tempi brevi e nelle vicinanze della propria abitazione.

L’entrata in vigore della nuova tariffa ambulatoriale, prevista per il 2026, modificherà gli importi rimborsati ai fornitori di imaging. Secondo gli elementi esaminati, ciò dovrebbe comportare una sensibile diminuzione della quota della tariffa riservata al finanziamento dell’infrastruttura degli istituti di radiologia.

Il CDF raccomanda perciò all’UFSP di monitorare l’impatto dell’introduzione della nuova tariffa, soprattutto per assicurarsi che ciò non porti a un aumento indesiderato del numero di esami.

Scarso impatto della regolamentazione cantonale per l’offerta di imaging

I Cantoni non sono rimasti inattivi di fronte alla crescente importanza dell’imaging. Su propria iniziativa, i Cantoni latini hanno introdotto una clausola del bisogno concernente le apparecchiature medico-tecniche pesanti, tra cui quelle per tomografie computerizzate e risonanze magnetiche. Tale clausola non ha avuto l’effetto moderatore previsto sul ricorso all’imaging. Infatti, è proprio nei Cantoni che hanno introdotto clausole di questo tipo che la frequenza di tomografie computerizzate e risonanze magnetiche rimane la più elevata e, al contempo, aumenta più rapidamente.

Alcune nuove disposizioni della LAMal prevedono che da metà 2025 i Cantoni applichino un limite al numero di medici esercitanti nei campi di specializzazione in cui l’offerta di tali prestazioni è superiore al necessario. La radiologia è una delle discipline mediche oggetto dei progetti di limitazione elaborati da più Cantoni. Poiché si prevede che i diritti acquisiti vengano garantiti, l’effetto di questa limitazione dovrebbe rimanere inizialmente modesto e potrà essere valutato soltanto fra diversi anni.

Gli audit sulla radioprotezione sono atti a migliorare la regolamentazione delle pratiche di imaging

Nel quadro delle sue competenze in materia di radioprotezione, l’UFSP conduce dal 2018 un progetto di audit clinici delle strutture che impiegano apparecchiature mediche a intensa radiazione, tra cui le tomografie computerizzate. Questi audit, condotte da colleghi specialisti, costituiscono l’unico strumento per controllare la qualità delle prescrizioni e dello svolgimento degli esami sulla base di casi concreti. Anche se riguardano unicamente le tomografie computerizzate, tali audit sono una misura supplementare che può migliorare la regolamentazione delle pratiche di imaging in Svizzera. Al momento, l’attuazione delle raccomandazioni emerse dagli audit non è oggetto di alcun monitoraggio sistematico e il loro impatto è difficilmente misurabile.

Nel 2025 il Consiglio federale verrà informato sullo stato di avanzamento dei lavori di attuazione di questo progetto. Il CDF raccomanda di introdurre un dispositivo efficace che consenta a chi gestisce le apparecchiature di verificare l’implementazione delle raccomandazioni.

La cartella informatizzata del paziente come strumento di guadagno in termini di efficienza

Si dà per scontato che un medico curante verifichi se il paziente ha già effettuato un imaging per evitare di ripetere un esame e di sottoporlo a irradiazioni nocive per la sua salute. Tale accertamento però non viene sempre eseguito a causa delle difficoltà di accedere a tali informazioni e del tempo ridotto a disposizione dei medici. La cartella informatizzata del paziente (CIP), volta a riunire i documenti medici più importanti, dovrebbe risolvere questo problema, ma è a oggi inefficace. Gli standard tecnici relativi all’imaging nella CIP sono stati definiti, ma in essa non si trovano né i rapporti radiologici né i link per le immagini perché gli assicurati e i fornitori non li inseriscono, dal momento che la cartella non è obbligatoria.

Dati gli evidenti guadagni in termini di efficienza e sicurezza, in occasione dell’annunciata revisione totale della legge federale del 19 giugno 2015 sulla cartella informatizzata del paziente (LCIP; RS 816.1), il CDF raccomanda all’UFSP di provvedere affinché sia garantito l’accesso integrale ai rapporti radiologici e alle immagini mediche tramite la cartella del paziente.
